LER SUPPLEMENTAL INFORMATION SQRO-50-327/80129 Technical Specification Involved 3.8.1.1 Reported Under Technical Specificaiton 6.9.1.13.c Date of Occurrence: 8/8/80 Identification and Description of Occurrence:

Diesel generator 1A-A failed to start during simulated loss of power to 2A-A 6.9-kV shutdown board. The 43T (L) switch for DG 1A-A was found in the test position which prevents the DG from starting on a loss of power, although the DG would still start automatically from a safety injection signal or manually.

Conditions Prior to Occurrence:

Unit 2 preoperational test TVA-13.B1 was in progress. The test called for a simulated loss of power to unit 2 "A" train equipment which would result in all 4 DG's starting and unit 2 "A" train equipment being sequenced in service.

Apparent Cause of Occurrence:

On 8/6/80, at 0816, the 1A-A DG 43T (L) switch was placed in test to perform SI-7 (DG Start from Simulated SIS). The 43T (L) switch was not placed back in the normal position according to the instruct-ions in SI-7.

Analysis of Occurrence:

A lack of communications between the RO and SRO caused each to think that the 43T (L) switch had been placed normal. SI-7 data sheet did not have a sign off to verify this action.

Corrective Action

SI-7 is being revised to include sign off spaces to place or verify DG's standby readiness upon completion of the surveillance. RO's and SRO's have been reminded to follow instructions as well as sign off's ou data sheets during performance of SI's.

SI-2 (Shift Log) has been revised to include a sign off verification of 43T (L) position.
