Combined Inspections 50-317/80-06 and 50-318/80-06 This refers to the inspection of activities authorized by NRC License Nos.

DPR-53 and DPR-69.

This inspection was conducted during the period May 1-31, 1980, by Mr. R. Architzel, the Senior Resident Inspector assigned to the Calvert Cliffs Nuclear Power Plant by the NRC Region I Office.

The findings of the inspection were discussed with Mr. L. Russell of the plant staff at the conclusion of the inspection.

Areas examined during this inspection are described in the Office of Inspection and Enforcement I.'spection Report provided under separate cover.

Within these areas, the inspection consisted of selective examinations of procedures and representative records, interviews with personnel, and observations by the inspector.

On May 2, 1980, the Calvert Cliffs Nuclear Power Plant, Unit 1, was operated for about three (3) hours in a condition which exceeded the Limiting Condition for Operation.

This condition rendered both Auxiliary Feedwater Pumps unavail-able for automatic start and supply of water to the Steam Generators for decay heat removal.

Although the Main Feedwater System remained operable during this time, the inoperability of the Auxiliary Feedwater System constitutes a serious matter which reveals an apparent weakness in control of equipment ess ntial to safety.

This inoperability was neither reported to the Commission nor logged as required.

This item is described more fully in the inspection report for the May 1-31, 1980 period of inspection of Calvert Cliffs Nuclear Power Plant.

Two additional items of noncompliance concerned failure to prevent activities posing fire hazards.

Further, three items of noncompliance were identified that related to security.

The items of noncompliance identi-fied during this inspection reflect weaknesses in the management of licensed activities in the areas of plant operations, fire protection, and security,i The Commission proposes to issue a civil penalty for the items of noncompliance set forth in Appendices A and B to this letter in the amount of Twenty-One Thousand Dollars ($21,000.00).

Appendix C is a Notice of Proposed Imposition of Civil Penalties.

e EcclosucCcatains 10 CE 2.790 htprial Bsitimore Cas and Electric Company M 1 1330 With respect to Item D in Appendix A, the NRC inspecter noted that BG&E Co. had corrected this noncompliance and therefore, in your response, only those actions taken to prevent recurrence of this item of noncompliance need to be discussed.

The Commission is particularly concerned about recurring errors involving non-licensed operating staff at the Calverts Cliffs plant.

Specifically, errors by non-licensed operators have resulted in unmonitored releases of non-radioactive waste water to the Chesapeake Bay as was discussed in a meeting held in the NRC Region I Office on Dec, ember 21, 1979.

Valving errors by Auxiliary Operators contributed to the Plant Emergencies which were the subject of a NRC Investigation Report forwarded to you by letter dated April 17, 1980.

BG&E Co. stated in a May 9, 1980 response to the report that effective April 18, 1980, an additional Senior Control Room Operator was added to each operating shift.

This operator will function as a work leader for plant evolutions outside the control room and will improve the ability to operate the plant in a safe manner.

It does not appear, however, that this added control has corrected a basic problem in operation of the plant.

On May 20, 1980, an error by a licensed operator in logging valve positions, compounded by another licensed operator using the incorrect entries for directing a non-licensed operator to reposition the valves, resulted in inoperability of the Auxiliary Feedwater System (AFWS).

The non-licensed operator then contributed to the inoperability of the Auxiliary Feedwater System in that he did not recognize that repositioning of the valves resulted in a loss of suction to the AFWS pumps, nor did he alert his supervisor that two of the three valves which he had been directed to reposition were already in the directed position.

In response to this letter, please give special attention to actions to ensure that operating staff performance is properly upgraded in the area of equipment lineup and status to minimize recurrence of errors in this area.

In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2, Title 10, Code of Federal Regulations, a copy of this letter and the Appendices A and C will be placed in the NRC's Public Document Room.

Additionally, the information in Appendix B concerns a subject matter which is exempt from disclosure under Section 2.790(d) of the NRC's " Rules of Practice,"

Part 2, Title 10, Code of Federal Regulations.

Accordingly, Appendix B and your response to the items listed in Appendix B will not be placed in the Public Document Room and will be disclosed only in accordance witn 10 CFR 9.12.
