Box 968 Richland, Washington 99352 Docket No. 50-460 Construction Permit No. CPPR-134 NOTICE OF VIOLATION Based on the results of NRC inspections conducted during the period of April 1-30,1980, it appears that certain of your activities were not conducted in full compliance with conditions of your NRC Construction Permit No. CPPR-134 as indicated below:

10 CFR 50, Appendix B, Criterion V, as implemented by Section 17.1.5 of the quality assurance program documented in the PSAR and the WPPSS QA Program Manual Procedure QAP-2, Paragraph 3.1, states, in part, that " Activities affecting quality shall be prescribed by documented instructions, procedures or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures or drawings...."

Detail T30 of the drawing for electrical tray support IT4-3404-300A specifies a nominal fillet weld size of 1/4-inch.

Detail T40D of the drawing for electrical tray support 1T6-3413-2910 specifies a nominal fillet weld size of 5/16-inch. The Foley-Wismer and Becker (electrical contractor) Quality Control Procedure SA states that the fillet weld size may underrun the specified nominal fillet weld size by 1/16-inch without correction provided that the underrun does not exceed ten percent of the length of the weld.

Contrary to the above, on April 11, 1980:

The horizontal beam to building-steel connection of support 1T4-3404-300A had a 3/16-ir.ch fillet weld over the full length of the weld instead of a 'i/4-inch fillet weld as shown in drawing detail T30.

2.

The diagonal brace connection to the lower gusset plate of support 1T6-3413-2910 had e 1/4-inch fillet weld over the full length of the weld instead of a 5/16-inch fillet weld as shown in drawing detail T400.

Both supports had been inspected and accepted by the contractor's inspectors.

This iten is a deficiency.
