Letter from G. S. Spencer to N. O. f,trand, NRC Inspection at WNP-1/4 Site, dated June 26, 1980.
The referenced correspondence delineated the results of the April 1-30, 1980 inspection of activities authorized by fiRC Construction Permits Nos. CPPR-134 and 174.
Further, the referenced correspondence identified a certain activity which was not conducted in fall compliance with PSAR requirements set forth in the Notice of Violation enclosed as Appendix A.
This item of noncompliance has been categorized into a level as described in your correspondence to all NRC licensees dated December 31, 1974.
This letter is submitted in response to the referenced correspondence. The specific NRC finding, as identified, and the Supply System response is provided herewith as Appendix A.

APPEllDIX A Nuclear Regulatory Comission Region V Suite 202, Walnut Creek Plaza 1990 N. California Boulevard Walnut Creek, California 94596 Docket No. 460 Construction Permit No. CPPR-134 NOTICE OF VIOLATION Based on the results of NRC inspections conducted during the period of April 1-30, 1980, it appears that certain of your activities were not conducted in full compliance with conditions of your NRC Construction Permit No. CPPR-134 as indicated below:

10CFR50, Appendix B, Criterion V, as implemented by Section 17.1.5 of the quality assurance program documented in the PSAR and the WPPSS QA Program Manual Procedure QAP-2, Paragraph 3.1, states, in part, that " Activities affecting quality shall be prescribed by documented instructions, procedures or drawings, of a type appro-priate to the circumstances and shall be accomplished in accordance with these instructions, procedures or drawings...."
Detail T30 of tne drawing for electrical tray support IT4-3404-300A specifies a nominal fillet weld si7e of 1/4-inch. Detail T400 of the drawing for electrical tray support 1T6-3413-2910 specifies a nominal fillet weld size of 5/16-inch. The Foley-Wismer and Becker (electrical contractor) Quality Control Procedure SA states that the fillet weld size may underrun the specified nominal fillet weld size by 1/16-inch without correction provided that the underrun does not exceed ten percent of the length of the weld.
Contrary to the above, on April 11, 1980:

The horizontal beam to building-steel connection of support 1T4-3404-300A had a 3/16-inch fillet weld over the full length of the weld instead of a 1/4-inch fillet weld as shown in drawing detail T30.
2.
The diagonal brace connection to the lower gusset plate of support 1T6-3413-2910 had a 1/4-inch fillet weld over the full length of the weld instead of a 5/16-inch fillet weld as shown in drawing detail T400.
Both supports had been inspected and accepted by the contractor's inspectors.
This item is a deficiency.
Appendix A Page 2 SUPPLY SYSTEM RESPONSE Corrective Actions Taken and Results Achieved A.1 Support 1T4-3404-300A - It should be noted that inspection on this support had not been completed by the contractor's organization and should have been considered as work in process, as final inspection had not been completed. The installing contractor initiated non-conformance report 1-CNCR-218-586 which has been dispositioned rework. Due to our present labor dispute, craft personnel ore not available at this time to perform the necessary rework.
A.2 Support IT6-3413-2910 - The installing contractor initiated non-conformance report 1-CNCR-218-555 which has been dispositioned rework. Rework has been accomplished and accepted by the contractors quality control organization on June 4,1980.
Project Quality Assurance selected at random ten (10) cable tray -
supports for reinspection. The ten (10) selected supports contained nine-hundred-twenty-eight (928) welds. Of the nine-hundred-twenty-eight (928) welds selected, one-hundred-eighty-six (186) had not been inspected or accepted by the contractor's quality control organization.
Nineteen (19) of this group were undersized. The remaining seven-hundred-forty-two (742) welds selected had been inspected and accepted by the contractor's quality control organization. Twenty-six (26) of the accepted group of welds were undersized.
Inspection results are contained in Surveillance Reports 1-218-115, 116, and 119. Deficient welds identified to date are contained in contractor originated nonconformance report numbers 1-CNCR-218-592, 600, 601, 602, 631, 632, 633, 634, 635 and 636.
The contractor is currently reinspecting fifty-five (55) cable tray supports in eleven (11) different areas of the plant.
Deficient welds will be identified through the contractor's nonconformance reporting system.
Corrective Actions Taken to Preclude Recurrence The installing contractor has conducted training sessions for welders, inspectors, production superintendents, supervisors, and engineers.
In addition, weld gauges have been issued to welders.
We will detennine if additional reinspections are appropriate pending a review of inspection results identified in paragraph A.2.
Similar conditions do not exist at WNP-4.
Date of Full Compliance The reinspection effort of the fifty-five (55) cable tray supports should be complete by August 1, 1980. Actual rework will be delayed until resolution of the existing labor dispute.
