LER SUPPLEMENTAL IhTORMATION SQRO-50-327/80126 Technical Specification Involved 3.6.1.9 Reported Under Technical Specification 6.9.1.13.c Date of Occurrence:

6/27/80 Idencification and Description of Occurrence:

The following containment purge valves were found open: FCV-30-14, 15,

56. 57, 37, and 40.

This provided two (2) paths from lower containment to the contain=ent purge exhaust system.

Conditions Prior to Occurrence:

Lower containment was being vented to the annulus using FCV-30-37 and FCV-30-40. The reactor operator was advised by the senior reactor cperator that he needed to place the purge system in service and cease the venting. This was an attempt to cause the ice condenser inlet doors to close by lowering pressure in the lower containment.

Apparent Cause of Occurrence:

The ' tactor operator failed to close the vent valves but did notify the results section of the change and the need to leak test FCV-30-37 j

and FCV-30-40.

, Analysis of Occurrence:

,i The mactor operator was not totally observant of equipment conditions l

in his haste to close the ice condenser inlet doors.

Corrective Action

Operator was cautioned to review currens >oeration of equipment before making changes. A discussion with the ope.ations supervisor was held on shift the following day. Further training is.being initiated for all operators.
