- 14. June 1984 Notice of Violation ud Civil Penalty was issued June 18, 1984, associated with violations regarding excessive heatup rates, an unplanned reactor pressurization, and excessive rod scram times (EA 84-39,$30,000).
- 15. June 1984 Order modifying license dated June 18, 1984, regarding violations associated with Enforcement Action 84-39 requiring the licensee to submit and implement a plan for an appraisal of:
(1) the process for performing safety evaluations and reviews of procedures pursuant to 10 CFR 50.59 to determine if the process is currently effective, or if improvements are needed; (2) plant and system operating procedures to verify that existing procedures are consistent with Technical Specification bases and of the FSAR and do not involve unreviewed safety questions; and (3) the program for ensuring that employees involved in the review and approval of operating procedere remain cognizant of the licensing bases.
- 16. July 1984 An Enforcement Conference was held July 31, 1984, to discuss the findings of Inspection 50-277/84-19 and 50-278/84-10 relative to security plan violations (EA 84-44, no civil penalty).
- 17. February 1985 An Enforcement Conference was held February 8,1985, to discuss the findings of Inspection 50-278/85-07, a Unit 3 event regarding simultaneous diesel generator inoperability and containment cooling.
- 18. March 1985 An Enforcement Conference was held March 4, 1985, to discuss ra'diological practices during the Unit 2 outage.

Four Severity Level IV violations were issued.

- 20. June 1985 Civil Penalty was issued June 7,1985, associated with NRC Inspection 50-277/85-11. The violations were associated with radiation protection practices during the Unit 2 pipe replacement outage.
The Notice of Violation and Civil Penalty were combined with escalated enforcement from Limerick (EA 85-42,$25,000).
- 21. June 1985 Enforcement Conference was held to discuss an apparent in-attentive Unit 3 reactor operator. No Enforcement Action was taken, the licensee was requested to describe actions to prevent recurrence.
(Inspection Report 50-278/85-22).
- 22. November 1985 Confirmatory Action letter dated November 5,1985, regarding actions to be taken by PEco in the area of Peach Bottom emergency preparedness.
- 23. November 1985 An Enforcement Conference was held on November 14, 1985 to discuss problems with radwaste transportation activities.
Two Severity Level III, two Severity Level IV, and one Severity Level V Violations were invclved. No civil penalty because States had taken penal actions.

Sleeping on the Job An allegation was made to me yesterday that some control room personnel at Peach Bottom have been seen at various times. in a state of inattentiveness to their duties in that they were giving the appearance, at least, of sleeping or attempting to sleep at their posts.
No names or times have been provided to me.

Therefore.you and your staf f, and shif t supervision are to see immediately and continually that no such allegation can ever be made factually against our personnel.
l In order that everyone may know the seriousness with which 1 view this situation the following policy is in effect:
r Anyone assigned to the control room operating staff who appears to be sleepirq at his post is to be relieved immediately and sent to the Plant Manager for administration of i
discipline.
Discipline will consist of immediate suspension with recomendation for termination. There are to be no exceptions.

$UBJECT: REMARKS SY J. L. EVERETT Friday afternoon April 3,1987, Mr. Everett took a short tour of the plant.
Before he left, he addressed some of the plant staff. He told us the following things about our shutdown order and the Company's response to it:
l This is the most serious event in his history with the Company. The Company i
is treating it as being as serious as a nuclear accident. We will do all we can to requalify for operation of the plant. The outcome is dependent on the. cooperation of all the people at Peach Botton.
His guess is that it will take months before we can return to operation.
In order to gather as mach information as possible, both at Peach Bottom and through the Company, a number of teams are being put together. There is a team within the Company. A special committee for the Boar.d of Directors is being proposed and Dennis Wilkinson. the first president of INPO will act as a consultant to that committee.

There will probably be a Pennsylvania state team and Mr. Everett will offer our information to the Governor of Maryland if he wishes to form a team, i
He cannot say if our jobs are at risk or not. He knows that his job is at risk.
He does anticipate that there will be a number of personnel reassignments.
He believes thac the problem lies in the cttitude of some people at Peach Bottom. The problem is not with the condition of the plant or our technical expertise. He believes that the great majority of people want to carry out their duties in a professional manner. He says there is no plac; at Peach Bottom or Philadelphia Electric Company for anybody who believes that he is above operation in accordance with procedures.
In answer to quest'icns', Mr. Gallagher said there is a possibility that the manpower commiitaent to the Unit 2 outage might be cut back. Mr. Kemper said we do not know how the Unit 3 pipe replacement outage will be a,ffected and that we won't know until the scope of this problem is identified.

MONITORING ACTIVITIES IN CONTROL ROOM j
g A primary part of the company's short term response to the NRC's shutdown order consists of the establishment of an independent team of engineers to monitor control room activities. These five individuals will report directly to Bob Moore, Superintendent of the Quality Assurance Division.
The purpose of this group. is:tormonitor. control: room. personnel and verify j
attention.to. duty stations. and-adherence to the. provisions of. Administrative E.
Procedure A-7.
These individual's have.no responsibility for technical matters,-
nor do tpey have any supervisory authority over any Philadelphia Electric personnel. This. five man group will be on shif t around the clock for a period of six to eight weeks. At this time, it is not certain whether or not the program will continue beyond that point with other personnel.
The five individuals in this group are: Lou Vernacchio, Peach Bottom Test Engineer; Paul Bushek Peach Bottom ISEG; Steve Breeding, Peach Bottom QC; Rich
- Weisband, Limerick Test Engineer; and Scott Auve, Nuclear fuel Section, j
At this point, it is imperative that the on-shif t monitoring activity succeed.
l We must treat these five individuals with respect and courtesy. This activity l
as well as many others will be carefully evaluated by the NRC.
it is the NRC's perception of our attitude that is a key f actor in our recovering from our present situation. The courtesy and respect with which we i
treat each other as well as individuals from outside organizations,-including the NRC will be closely scrutinized. Currently, the NRC has the perception that we are discourteous and unreceptive to any outside individual or group. They see this as evidence of our cavalier attitude toward nuclear safety. Whether or not we agree with this latter perception is unimportant. We must convince the NRC of our commitinent to nuclear safety.
~Therefore, we must treati each other as well as everyone else within the Peach Bottom community, with respect, dignity, and courtesy. We must overcome the appearance of hostility towards other groups and individuals.

2 Our technical competence with respect to control room operations has never been i
questioned and ismot.being questioned. now. Our basic problem is the NRC's l
perception of our casual attitude toward nuclear plant safety.
This is indicated to them by allegations of operators sleeping in the control room, by several instances of failure to follow procedures, and by the lack of cooperation some NRC inspectors have experienced with some station personnel.
We must all recognize that we can not go back and undo the past.
From this time forward, we must strive to build an attitude of mutual cooperation and trust i
with all of the people with whom we dJ business on a daily basis. We must i
strive to present ourselves to others as the professionals we really are.
n in all plant operations but in particular in control room activities, a professional demeanor is important at all times.
Professional conduct in the control room should be evidenced by not placing food or beverages on control panels, by keeping feet of f of control panels or desks at all times, and by strictly adhering to the Nuclear Plant Rules Procedure A-7 and any other appropriate procedures. But above all, professionalism is demonstrated by the way we act towards one another, for exampic, when it is
..appropr4 ate to limit.the number of. personnel in the. control.. room, it can be doner-in such a manner that the individual on group being asked to leave understands 4_
why they must leave and not so they are made to feel like second class citizens.--

Again, treat people with respect and dignity at all times.
In a continuing ef fort to improve comunications, the monthly meetings with the operator group and the control room supervisor group will be accelerated. An agenda for each meeting will be published by the end of this week.
I know I've repeated this phrase several times in this letter, but I am absolutely convinced that the only way Peach Bottom can recover f rom our present predicament, is for all of us in the Peach Bott,om comunity to learn to treat each other with respect and dignity regardless of who signs our paycheck. This is not to say that we won't disagree, but we must recognize that we are on the same team and l
all striving for the same final goals.
We must appreciate that this is not an operator problem, or a management problem or an NRC problem, but it is in fact a Peach Bottom problem.
In each one of us there some responsibility for the solution.
