conclusien of the inspection.

the enclosed inspection reper:. Within these areas, the inspection
consisted of selective exacination of procedures and representative
records, interviews with personnel, and observations by the inspector.
Eased on the results of this inspection it appears that certain of yeur
activities were not conducted in full compliance with l3C requirements.
These inces and the references to the pertinent requirements are lissed
in Appendix A of this letter.

This noti:e is sent to you pursuant to the previsions of Section 2.201
of the :aC's " Rules of Practice".
Section 2.201 rec;uires you to submit
to this office, within 20 days of your receipt of this netice, a written
state 2nt of explanation in reply, including:
(1) ccrrective steps which
have been or will be taken by you, and the results achieved; (2) corrective
steps rhich will be taken to avoid further violations; and, (3) the data
when full compliance will be achieved.
