Mis refers to the inspection conducted by Messrs. Jones and Hayes of this office on June 8-10, 1971, of construction activities at the Davis-Besse construction site authorized by ABC Construction Permit No. CPPR-80 and to the discussions of our findings at the conclusion of the inspection with Messrs. Lenardson, Eichenauce, Moring and others of your staff.
Areas examined during the inspection included roccur additions to the Toledo Edison Company CECO) Quality Assurance staff, concreto reinforcement and placceent activities, fabrication of the spent fuel pool liner, fabrication of the reactor containment structure, and a review of provisions to assure that thickness measurements or class I valve body castings have been made o that drawing and calculational requirements can be verified. Within these arcas, the inspection consisted oi selective examination of procedures and representative records, interviews with plant personnel, and obserysticas by the inspectors.
During the inspection, it was found that certain of your activities 1
appear to be in noncomformance with 10 CFR 50, Arpendix B.

Please provide us within 30 days, in writing, with your comments concerning these items, any steps which have been or will be taken to correct them, any steps that have been or will be taken to prevent recurrence, and the date all corrective actions or preventive measures smre or will be completed.

September 3, 1971 Witn regard to questions raised during the inspection concerning the following:
(1) a possibility that the shield building wall plumbuess is not within the tolerance limit of the specifications, a..d (2) provisions to assure that Class I valve casting wall thickness measurements have been made so that drawing and calculational require-ments can be verified, we understand tuat additional information and data will be collected and made available for our review. We will examine these matters further during our next routine inspection.
If you have any questions concerning this inspection, we will be glad to discuss them witu you.

ENCLOSURE DOCKET No. 50-346 Certain activities under your construction pensit appear to be in nonconformance with 10 CFR 50, Appendix 2, as indicated below:

10 CFR 50, Appendix B, Section VII, states, in part, that "
Measures shall be established to assure that purel sed material,

The Bechtel Technical Specification No. 7749-C-25, Paragraph 6.2.3, states in regard to Fly Ash testing: ".
The certified mill test report must be approved by the Construction Manager to qualify the supplier and commence delivery to the job site.
Each subsequent delivery shall be accompanied by a statement certified by the supplier attesting that the Fly Ash produced in a continuous fashion is sampled for chemical and physical tests at the representative The Nicholson rate indicated in specification ASTM C311-68."
Concrete and Supply Company QA/QC manual, Pages 4 and 5, states, in part, that :
"This fly ash conforms to specification ASTM C-618 Class F.
The N31 son's plant superintendent will not permit the unl.t_., or any delivered materials prior to receiving two certified copies of the mill test report."
Contrary to the above, Fly Ash is delivered to the site and used without receipt of the required certifications.

10 CFR 50, Appendix B, Section V, states, in part, that :
" Activities
[
affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings."
Contrary to the above, documented instructions were not followed on one occasion when one load (nine cubic yards) of concrete, designed for use in the reactor shield building, was inadvertently placed in the spent fuel pit floor slab. While the mix design of the mis-this situation placed concrete was acceptable for either location, was not detected even though the concrete delivery ticket was signed by both the testing laboratory representative at the batch plant.and l
