Identification and Description of Occurrence:

During the performance of SMI-1-D/G on D/G 2A-A, the operator mistakenly tripped the normal feeder from 6.9-kV unit board 2B-B.without putting the synchroscope switches to "0N" position.

This caused loss of power to 6.9-kV shutdown board 2A-A.

The blackout signal caused all 4 D/G's to start. During the recovery all D/G's were made inoperable for GE 10 minutes to reset relays.

Conditions Prior to Occurrence:

0 Unit 1 in Mode 3 RCS Tavg = 545 F Pressurizer pressure = 2235 psig.

Apparent Cause of Occurrence:

An unclear specific main:enance instruction directed the operator to inadvertently de-energize the shutdown board. This caused all fcur D/G's to start for a blackout signal. The operator failed to recognize this as a station blackout condition and refer to E0I-5 and he subsequently emergency tripped all four D/G's.

A ten minute period when all D/G's were inoperable occurred while all lockout relays were being reset.

Analysis of Occurrence:

There was no danger to the public health and safety because there is no radiated fuel onsite.

Corrective Action

The SMI is being reviewed and revised to prevent a recurrence of inadvertent de-energizing of a shutdown board.

Specific steps and signoff will be inserted to prevent this. A section is being added to shutdown board SOI's adapting the station blackout recovery from E01-5 to be used as'a recovery instruction on any emergency start of the D/G's.

This instruction leaves the diesels running until all relays are reset.
