UNIT SHUTDOWN AND POWER REDUCTIONS EXPLANATION SHEET DOCKET NO. 50-338 REPORT MONTH April UNIT NAME North Anna 1 YEAR 1980 DATE 5-8-80 COMPLETED BY 703-894-5151 ext. aus 80-12 (H) 4-3-80 Time: 2312 the performance test scheduled Operations Department to perform a 3 stroke test on the three main steam trip valves located in the main steam safeguards building.
The valve is automatically supposed to close down only by 3 to verify operability each 92 days, however, it traveled past this point and closed completely.
High steam flow resulted in remaining 2 lines with accompaning low pressure causing safety injection & reactor trip. The 3 MSTV's were inspected and their 0
3 limit reset.
The test is now no longer required to be performed while at power.
80-13 (G) 4-8-80 Time: 1147.. turbine' trip / reactor trip.
Operator in turbine building was investigating the cause of a High level alarm in the.EHC 1
reservoir. The actual level was normal.
He attempted to cycle the alarm switch to see if it was sticking but he was on the Lo-Lo alarm switch.
Upon Lo-Lo alarm both EHC pumps tripped lock-out causing low EHC pressure signal to trip the turbine.
