Event Description

i The 2B RHR heat exchanger lower head was removed to inspect the RHR service water piping and the connection to the heat exchanger (internal inspection).

While inspecting, the pass rib plate was found damaged.

The pass rib separates the RHR SW entering the heat exchanger from the RHR SW 1eaving the heat exchanger after it has passed through the U-shaped tubes. The pass rib is welded to the tube sheet at the top, welded to the channel assembly on both sides, and fits into a groove in the channel cover at the bottom. The pass rib is 1" thick x44-3/4" (high) x54-1/2" (wide) and is made of SB-402, Alloy #715, (70-30 cu N1) material.

The plate was buckled in the center at the bottom where it fits in the groove in the channel cover.

The plate was off-center approximately 8.5 inches at the widest point.

The welds up each side of the plate were pulled loose to within 8-10 inches of the tube sheet. A small amount of the service water flow was allowed to pass under the plate instead of through the heat exchanger tubes. This was not noticeable in the performance of the heat exchanger.

Corrective Actions

1.

Section A.2.6 of Operating Procedure OP-43, Service Water System, is being i

revised to require venting the system whenever the system is filled.

2.

The auxiliary operator's Daily Surveillance Report is being revised to include a sign-off verifying that the RHR System is' filled and vented.

The system will be vented at least twice weekly with the time required to complete the venting noted. After six months, the venting requirement will be evaluated to determine if a continuation is required.

3.

All Operating Work Permits which have a potential for admitting air into the system will be revised to require that they be filled and vented prior declaring the system operational.
