O APPENDIX A NOTICE OF VIOLATION Duquesne Light Company Docket No. 50-334 Based on the results of an NRC inspection conducted on December 12-14, 1979, it appears that one of your activities was not conducted in full compliance with conditions of your NRC Facility License No. DPR-66.
This item is a deficiency.
Technical Specification 4.7.9.1 requires that safety related hydraulic snubbers listed in Table 3.7-9 be demonstrated operable by performance of functional tests to verify correct piston movement, lock up and bleed.
Technical Specification 6.8.1 states, in part, that " Written procedures shall be established, implemented and maintained..."
Station Administrative Directive No. 8, " Adherence to Procedures" states, in part, that "All station employees shall conduct operating, maintenance, test,
... activities which affect the QA Cat _egory I systems, structures, and components in accordance with the required approved procedures.
Corrective Maintenance Procedure No. 1-75-159, " Operating and Maintenance Pro-cedure for ITT Grinnell Snubber Tester", Rev. O,Section IV.B.14 requires that the hydraulic fluid temperature be measured and recorded on Test Report Form PHD-5434-F.
Contrary to the abose, procedures were not implemented in that:
During the Technical Specification required Functional Testing of safety related ITT Grinnell Snubbers f rom July 5 to July 18, 1979 the hydraulic fluid temperature was not measured or recorded on the Test Report Form PHD-5434-F.
