Criterion XVI, " Corrective Action," states in part, ".

Centrary to the above, and over the verbal "QA Hold" order placed en the da= aged ec=ponent, repair of are strikes on one no::le of the reactor vessel was cen=enced prior to receipt of a docu=ented repair procedure.
There was no evidence that Luke has a procedure for documenting a.d reporting defects to proper levels of =anage=ent, or for assuring the effectiveness of "QA Holds" cn noncenforming equip =ent pending adequate engineering dispcsitien.
