On May 8,1978, you reported a recent shaft failure in decay heat removal pump (DHP) 1-A at Crystal River Unit No. 3 (CR-3). As noted this was the second such failure of this pump.

The DHPs are part of your emergency core cooling system and are intended to supply low pressure safety injection and long tem cooling in the event of a loss of coolant accident. The shaft failures which have occurred place the reliability of the DNPs at CR-3 in question. There-fore, you are requested to justify that your DHPs can be expected to perform their intended functions. This justification should include a description of the shaft failures, DHP operating histories. DHP operating requirements, corm ctive action taken or to be taken and additional surveillance currently perfomed and proposed. In addition you should i

provide your detemination, including analytical support, of the cause of the failures and a clear sectional assembly drawing of the DNP, with dimensions. Specific reference to previously submitted material is j

acceptable.

Due to the safety significance of this issue we must agree. prior to l

i restart of your facility, that the DHPs at CR-3 can be expected to l

perform their intended functions. Therefore, please submit the requested infomation as soon as possible.
