The attached report of an inquiry from the subject licensee is

You will note that we were not informed of this incident until some
60 days following the occurrence. We attribute the eventual reporting
of this matter to renewed awareness on the part of the licensee with

respect to the requirements of 10 CFR Part 50.55(e).
In any event,

the licensee has stated that a written report will be submitted on

an expedited basis.
We plan to review the circumstances associated with this incident,
including late reporting in terms of enforcement action, during the
next routine site inspection.
