On July 27, 1972, we reported to you an incident concerning shipping damage to a reactor coolant pu=p casing for the Davis-Besse Tuclear Power Station, stating that we had not considered the incident reportable under 10 CFR O
Part 50.55(e), but that your repruentatives had requested a fer=al report.

A danage incident occurred to the equip =ent hatch for the contain=ent vessel which we had not considered to be of a sufficiently significant nature to be reportable under 10 CFR Part 50.55; however, since this incident is si=ilar to the one involving the coolant pu=p case, we are for-ally reporting this damage.
The enclosed report, dated August 3,1972, covers details of shipping da= age to the equipment hatch for the containment iressel which was da= aged in transit on a railway shipment. This report also covers details of the shop inspection and repair at the manufacturer's plant to ensure that this co=ponent =eets all conditions of the codes and specifications under which it was fabricated.

Report of Shipping Da= age to Containment Vessel Equipment Hatch.
Background
The containment vessel for the Davis-Besse Station is being fabricated and field-erected by the Chicago Bridge & Iron Co=pany and is in final stages of erection.
The 13-foot diameter equipment hatch for this structure had been shipped to the job site on April 11, 1972. After shipment, a routine internal CB&I shop audit disclosed that velded attachments had been attached after stress reliev-ing, which required further stress relieving.
-On-May 2h,1972, this equipment hatch was shipped from the site to the Chicago

plant for stress relieving in the shop facilities.
NJ Incident On May 28 or 29, 1972, while the shipping car was on the Penn Central Railroad
'at Elkhart, Indiana, the shipping car was hu= ped sufficiently to damage the equipment hatch. Upon arrival at the C3&I Chicago plant, inspection revealed damage to the extent that repair vould be necessary.
Inseection and Retair A complete inspection and repair procedure with results of da-nge was submitted under date of June 6,1972, and approved on June 9,1972.
The damage consisted of two dented-in areas of the dished door head. The en-tire head was cleaned of paint, the veld sears and attachment velds were sand-blasted and =agnetic particle tested.

out and replaced with a new dished center plate.
The entire equipment hatch assembly was then stress relieved, final inspected, and finished for shipment.
l The inspection, repair, and stress relieving was completed and the hatch assembly re-shipped to the job site on June lh, 1972. Installation of the hatch assembly in the containment vessel commenced on June 26, 1972, and is essentially co=plete.

repaired using detailed approved procedures in the =anufacturer's plant and the hatch meets all requirements of the codes and specifications under which it was manufactured.
