1330 CST Unit 1 Identification and Description of Occurrence The Chemical and Volume Control System (CVCS) centrifugal charging pumpn l

could fail due to inoufficient flow delivery during a postulated secondary side high energy line rupture with coincident failure of the power-operated relief valves. Incufficient flow could rcsult when the Reactor Ceolant System (PCS) reaches a presnure of approxirately 2500 psi. Under this sequenen of events, the mini flow valves for these pumps are closed. Therefore, the centrifugal charging pumps could deadhead against the RCS and burnout.

Conditions Prior t_n Oc_c_ur_rence Unit 1 in Mode 5.

Actions specified in the Technical Specificatienc Surveillance Requirencnts not n.et due to Incycrable eq itpecnt.

N/A Apparent Cnuse of_0ccurrence_

Deficiency in r,yaten desirn. Westinghouse previded a centrifugal charging perp with insufficient discharge head or they negicceed to provide pressure relief on the discharge side of the pump.

Analyafe of Occurrence There is no danger to the health and safety of the public.

Corrective Action

Emergency Operating Instructions protect the centrifugal charging purps from burning out.

Westinghouse and TVA Engineering Pesign are currently evaluating design changes for permanent corrective action.
