LER SUPPLEMENTAL INFORMATION SQRO-50-327/ 8062 Technical Specification Involved 3.6.1.9 Reported Under Technical Specification 6.9.1.13.b Date of occurrence: 5/13/80 Time of occurrence:

1245 Unit 1 Identification and Description of Occurrence:

Two (2) pairs of containment purge system lines were discovered open. The operator was purging lower containment per SOI-30.2A.

An instruction deficiency allowed the operator to open all lower purge valves.

Conditions Prior to Occurrence:

Unit 1 in Mode 3 RCS SE 3350 F and pressurizer pressure 2235 psig.

Apparent Cause of Occurrence:

Instruction deficiency.

Analysis of Occurrence:

There was no danger to public health or safety.

Corrective Action

The operator was assigned to make a revision to the instruction to clarify the intent of using only one (1) line for purging.
