The occurrence is the failure to perfom a surveillance test at the required frequency.

Description of Occurrence:
At approximately 1300 hours0.015 days <br />0.361 hours <br />0.00215 weeks <br />4.9465e-4 months <br /> on March 31, 1977, it was discovered that the 4.71 C1 Pu-Be Source had not been leak tested in the last 6 months in accordance with Technical Specification 4.14 prior to use by ISC l
personnel during the period 3/12/77 to 3/15/77. The sealed source leak test was due to be perfomed on 12/26/76 the the +2S% allowance would have expired on approximately 2/4/77.

Analysis of Occurrence:
Since the source was leak tested satisfactorily and there was no removable surface contamination found, no hazard to the health and safety of the public existed.

Tlw source was leak tested on March 31, 1977 and the importance of

There has been no other occurrence reported due to the failure of the Health Physics Group to perfo m surveillance tests at the required frequency.
