The occurrence is the failure to perfom a surveillance test at the required frequency.

._. 7. Description of Occurrence: -

At approximately 1300 hours0.015 days <br />0.361 hours <br />0.00215 weeks <br />4.9465e-4 months <br /> on March 31, 1977, it was discovered that the 4.71 C1 Pu-Be Source had not been leak tested in the last 6 months in accordance with Technical Specification 4.14 prior to use by ISC l personnel during the period 3/12/77 to 3/15/77. The sealed source leak test was due to be perfomed on 12/26/76 the the +2S% allowance would have expired on approximately 2/4/77. -
