In accordance with Technical Specification 6.12.3.2, we have attached the subject reports concerning the failure to perfom a proper sur-veillance test on the Reactor Building Escape Hatch at. the required frequency and the failure to perfom a leak test on a Pu-Be source within six months prior to its use.
