Gentlemen:

This refers to the inspection conducted by Mr. Dodds and other members of

this office on March 9, 20-24, and April 6,1972, of construction activities

authorized by AEC Construction Permit No. CPPR-56, and to the discussion

held by Mr. Dodds with Mr. Mattimoe and other members of your staff, and

Bechtel project representatives, at the conclusion of the inspection.

Areas examined during this inspection included (1) the nondestructive testing

techniques used for the constructien of the containment vessel liner, Class I

site-fabricated vessels, and Class I piping other than primary system piping,

(2) your QA program, (3) plant operating organization and personnel training,

(4) operations pertaining to the placement of the reactor vessel, (5) receiving,

storage, and installation of selected Class I ct,mponents, and (6) a lack of

fusion condition in the root pass of stainless steel pipe welds.

Within

these areas, the inspection consisted of selective examinations of procedures

and representative records, interviews with project personnel, and observations

by the inspectors.

The inspection also included an examination of tha actions you have taken

with respect to the items identified in our letter of January 14, 1972. We

have no further questions concerning those items.

During this inspection, it was found that certain of your activities appeared

to be in nonconformance w2.th AEC requirements. The items and references to

the pertinent requirements are listed in the enclosure to this letter.

Please

provide us within 30 days, in writing, with your cormnents concerning these

items, any steps that have been or will be taken to correct them, any steps

that have been or will be taken to prevent recurrence, and the date all

corrective actions or preventive measures were or will be completed. Your

comments in response to the second item in the enclosure should include

information on the bases used for your acceptance of the containment vessel

liner welding as conforming to ASME Code requirements.

It is our understanding, with respect to the following items, that you intend

,

to take the actions indicated:

,

1.

A report concerning the discovery of a lack of fusion condition in

the root pass of stainless steel pipe welds will be submitted to the

Division of Compliance by May 30, 1972.

2.

The bulging (inward displacement) and the out-of-plumb condition of the

containment vessel liner place will be re-evaluated and reported as

appropriate to the Division of Compliance in accordance with the new

reporting requirements of 10CFR50.55, adopted April 1972. It was

noted that the inward displacement of the liner between liner anchors

exceeded the parameters used in your original analysis for acceptance

of the bulging.

3.

Grinding operations of carbon steel and stainless steel fittings will

be examined and precautions taken to eliminate the potential for

contaminating stainless steel end-preparations with carbon steel

These items will be examined during the next inspection.
