Sacramento Municipal Utility District Docket No. 050-0312 Certain activities under your construction permit appear to be in nonconformance with statements in your Final Safety Analysis Report, as identified below:

Section 1B.13 of Appendix 1B of the Final Safety Analysis Report and 4'

your Quality Assurance Procedure Number 17 require that all nonconforming material shall be segregated from acceptable material, documented on a nonconforming report, and dispositioned by proper authority.

Contrary to the above requirements, pipe flanges, bearing white tags with the words "Do Not Use on llN1", were intermingled with similar type Class I pipe flanges on a wooden pallet in the welding sh,p.

Apparently, these flanges had been rejected as Class I components but a Nonconforming Report had not been prepared nor were they identified with a proper QA tag.

Section 5.J.10.23 of Addendum 5 to the Preliminary Safety Analysis Report states, in part, that seam weld porosity in the containment vessel liner would be controlled as follows:

"...The seams will also be checked by 10% radiographic inspection. The criterion for radiographic acceptance of welds will be in accordance with paragraph UW-51 of Section VIII of the ASME Code, except that the maximum acceptable length of slag inclusion will not exceed 0.125 inch, whereas the code allows s

0.250 inch."

This requirement is further confirmed by Section 5.6.3.4.5.A.2.

of the Final Safety Analysis Report.

Contrary to the above requirement, fif teen radiographs of the twenty-five examined appeared to indicate that the welds may not meet the code require-ments for one or more of the following reasons: er.cessive porosity, linear aligned porosity, size of porosity, lincar indications, transverse cracking indications, artifacts, excessive slag inclusion lengths, undercut masking viewability, excessive weld splatter masking viewability, shim masking viewability, and nonfusion.
