Tcnne cca Vellsy Authority Sequoyah Nucisar Plcnt LER SUPPLEMENTAL INFORMATION SQRO-50-327/ 8017 Technical Specification Involved 3.3.3.9 Reported Under Technical Specification 6.9.1.13.b Date of Occurrence: 3/10/80 Time of Occurrence: 1030 CST Unit 1 Identification and Description of Occurrence:

At 1030 CST, 3/10/80, the Turbine Building Sump Monitor, 0-RM-90-212, was declared inoperable in order to work on flow switch FS-90-212.

This is contrary to LCO 3.3.3.9 requiring this monitor be operable.

Conditions Prior to Occurrence:

Unit 1 in Mode 6 following initial core loading.

i Action specified in the Technical Specification Surveillance Requirements met due to inoperable equipment. Describe.

SI-414, Turbine Building Sump and ERCW Discharge Radioactivity, was initiated to satisfy Action 29 of Table 3.3-12.

Apparent Cause of Occurrence:

Malfunction of flow switch FS-90-212. This switch initiates an alarm if flow through the radiation monitor is not detected.

Analysis of Occurrence:

There was no hazard to the public health or safety.

Corrective Action

Flow switch was replaced.

Failure Data:

Switch was supplied by General Atomic, manufactured by Delaval Turbine, Inc.,

Gems Sensors, Part No. 25364. There have been no previous failures.
