Areas Inspected:
Inservice inspection (ISI) program, implementing proce-
'
dures, observation of work activities, and review and evaluation of non-
destructive examinations (NDE) data and reports of ISI; review and
evaluation of radiographic examination results related to IE Bulletin 79-13; review of data reports of (NDE) examination related to IE Bulletin 79-17.
The inspection involved a total of 21 on site hours by one NRC
inspector.
Results: Of the six areas inspected, no apparent items of noncompliance
were identified in five areas; one apparent item of noncompliance was
identified in one area (infraction - failure to follow NDE procedures -
paragraph 4).

Observation of Activities
Inservice Inspection (ISI) Unit 2
The inspector observed liquid penetrant examination of pressurizer
nozzles on line No. 6-2-RC-20A and 6-2-RC-20B identified on NAVC0
drawing No. A7636(8) and determined that procedure No. NSP-PT-1,
Revision 1, was that not being followed. This is an infraction
identified in Appendix'A of this report.

Eddy Current Examination (ET)
The inspector observed the collection of ET data by W personnel of
steam generator 22.
The examination program identified a total of 499 tubes to be examined
in the hot leg of generator No. 21, two (2) tubes were identified
as having more than 70% wastage and were plugged. An expanded

examination program identified 427 tubes to be examined in the cold
leg. Nine (9) tubes were identified as having greater than 20% wast-
age and thirty (30) with less than 20%.
Examination of steam generator 22 identified 1079 tubes to be examined
on the cold leg of which four (4) were identified with greater than
50% wastage, eight (8) with greater than 45% and 52 with less than
20% wastage.
575 tubes are scheduled for examination on the hot leg
of generator 22.
The licensee held a meeting with W and determined
to pull a tube in generator 22 for analysis. A report of examination
results will be submitted to the NRC.
Except as noted, no items of noncompliance or deviations were identi-
fied.
