Tennessee Valley Authority Sequoyah Nuclear Plant LER SUPPLEMENTAL INFORMATION SQRO-50-327/80 03 Technical Specification Involved 3.3.3.10 Reported Under Technical Specification 6.9.1.12.b Date of Occurrence: 3/10/80 Time of Occurrence: 0730 CST Unit 1 Identification and Description of Occurrence:

For three hours and fifteen minutes sampler flow through the Iodine and Particulate filters in the Auxiliary Building Ventilation System Gaseous Effluent Monitor was not maintained. This is in violation of LCO 3.3.3.10 Action 36.

Grab samples were obtained as required by Action 34.

Conditions Prior to Occurrence:

Unit 1 in Mode 6 with initial fuel loading complete.

Action specified in the Technical Specification Surveillance Requirements met due to inoperable equipment. Describe.

N/A Apparent Cause of Occurrence:

Misunderstanding of Technical Specifications requirements.

Analysis of Occurrence:

There was no hazard to the health and safety of the public.
