The attached report by our field inspector of a visit to the subject

facility on March 4-5, 11-13, 18-20 and 22, 1974 is forwarded for

information. No violations were identified during the inspection.

The report includes observation of an unsuccessful lift-off test The

performed on a tendon associated with the containment building.

,

technique used to test the tendon, and the results achieved, indicate

that repeated testing.of a given tendon may cause progressive damage.

L. L. Beratan, Senior Structural Engineer, Technical Assistance Branch,

,

observed the test and has discussed the breaking of the wires with DL.

We have been informed by Karl Seyfrit, Chief, TAB, that Licensing will

'

evaluate the generic implication of this mode of failure and its effect

'

on the long term inservice surveillance program. .In addition, it is

our understanding that Beratan intends to follow the licensee's proposed

test program designed to determine the effects of repetitive testing of

a given tendon.

We also find that the licensee's efforts to improve their QA program

for operation is developing satisfactorily. We will continue to

monitor the implementation of this program.
