5. Identification of occurrence:

Failure of High Pressure Injection Isolation Valve (MUV-24) to open on

6. Conditions Prior to Occurrence:

Plant operating in Mode 3 following Reactor trip. .

7. Description of Occurrence: f, At 1356 on 22 March 1977, following a Reactor trip, while in the progress of maintaining RCS level and pressure, manual high pressure injection was initiated on HPI Train "A". One of two Train "A" High Pressure Injection Isolation Valves (MUV-24) failed to open. Train "A" MUV-23 and Train "B" MUV-25 and MUV-26 were available and operable.

8. . Designation of Apparent Cause of Occurrence:

The cause of thin event was the torque switch setting being too low for consistent opening of valve. MUV-24 tested satisfactorily on 23 March 1977.

, 9. Analysis of Occurrence:

There were no safety implications as either HPI subsystem in conjunction with core flooding system would maintain temperatures within prescribed limits in the event of accident conditions. Redundancy was maintained.

10. Corrective Action:

The torque switches were set higher to assure opening of the valve.

11. Failure Data:

This oenurrence was non-repetitive.
