Gentlemen:

This refers to the investigation conducted by T. F. Westerman, Blaine

Murray, G. D. Brown and H. Gaut during January 23-26, 1978, regarding

an unplanned radioactive release and to the discussion of the

investigation finding held by the investigators with Mr. R. Walker and

other members of his staff at the conclusion of the investigation on

January 26, 1978.

Areas examined during the investigation and our findings are discussed

in the enclosed investigation report.

Within these areas, the investi-

gation consisted of selec.tive examination of procedures and representa-

tive records, interviews with personnel, and observations by the

inspectors.

Based on the results of the investigation, it appears that certain of

your activities were not conducted in full compliance with NRC require-

ments. The items and references to the pertinent requirements are listed

in Enclosure 1 to this letter.

In particular, your attention is called to

the apparent failure to make timely initial notification of the two primary

off-site contacts as required by your Emergency Plan.

This letter constitutes a notice sent to you pursuant to the provisions

of Section 2.201 of the NRC's " Rules of Practice," Part 2, Title 10,

Code of Federal Regulations.

Section 2.201 requires you to submit to

Pending further investigation of the equipment malfunctions which occurred
