DUKE POWER COMPANY OCONEE NUCLEAR STATION - UNIT 1 UNUSUAL EVEN! REPORT UE-269/73-6 IMPROPER REVIEW OF A STATION MODIFICATION Introduction on June 28, 1973, a repair was made to an incore instrument tube without prior approval by the Station Review Committee (SRC), and review and approval by the Station Superintendent and the Assistant Vice President of the Steam Production Department.
This has been classified as an unusual event pursuant to Section 1.9c of the Oconee. Unit 1 Technical Specifications.
Description of the Incident On June 25, 1973, B&W issued Field Change 248 for the repair of the leaking incore detector tube.
This field change was reviewed and approved by the Design Engineering Department of Duke Power Company on June 26, 1973, and this approval forwarded to the Oconee Nuclear Station.
On June 28, 1973, a representative for the vendor of the incore detectors proceeded with the repair of the incore detector tube.
However, due to mis-routing of the Design Engineering approval, this modification did not receive the necessary review by the Station Review Committec and approval by the Station Superintendent and the Assistant Vice President of the Steam Production Department as required by Oconee Nuclear Station Administrative Procedure No. 10.
Subsequently, it was discovered that the vendor representative did not have documentation of his qualifications to make a weld repair to a pressure boundary of the reactor coolant system.
Corrective Action Currently, a study is underway within the Steam Production Department to determine the best method to assure that station modifications receive the J
proper review before implementation.
The results of this study will be incorporated in a revised administrative procedure for handling station modifier.tions. Presently, all vendors are being informed that no modifi-cations to the Oconee Nuclear Station shall be made without the prior 7912120[7f
t approval of the Station Superintendent.
Upon recommendation by the Station Review Committee, the weld made by the vendor representative was removed and the incore detector tube was re-welded by a qualified welder.
Safety Analysis This incident has safety significance in that the failure to properly follow Administrative Procedure No. 10 allowed a station modification to be made v5.thout adequate quality control. Prompt action by station management upon discovery of this incident afforded ample time to take proper corrective action.
