b 50-445/79-31 50-446/79-29 Appendix A NOTICE OF VIOLATION Based on the results of the NRC inspection conducted on December 18-20, 1979, and January 15-18, 1980, it appears that certain of your activities were not conducted in full compliance with the conditions of your NRC Construction Permit No. CPPR 126 as indicated below:
Failure to Follow Welding Procedures 10 CFR Part 50, Appendix B, Criterion V requires that established instructions, procedures, or drawings be followed for all activities affecting quality.
Brown and Root Weld Procedure Specification 88023, Revision 8, Interim Change Notice 1 requires that type 316 weld material be used when welding type 316 base material to type 304 base material.
Contrary to the above:
The IE inspector observed, on January 15, 1980, that type 308 weld material was being used to tack weld type 316 base material to type 304 base material for a Class 1 socket weld, identified as FW-13 on Drawing 3RP-SI-1-RB-025, Revision 1.
This is an infraction.
