Inspection Summary:
Inspection on November 26-30, 1979 (Report No. 50-266/79-18; 50-301/79-19)
Areas Inspected: Routine, announced inspection of confirmatory measure-
ments, environmental protection, and emergency planning for both units, in-
cluding licensee programs for quality control of analytical measurements;
submission of spiked air particulate and charccal adsorber for future
comparison; quality control of environmental measurements; implementation
of environmental protection program; coordination with offsite support
agencies; maintenance of emergency equipment, facilities, and supplies;
medical arrangements; training; emergency procedures; conduct of tests and
drills. The inspection involved 33 inspector-hours on site by one NRC
inspector.
Results:
Of the 15 areas inspected, no apparent items of noncompliance
or deviations were identified in 14 areas; an apparent item of noncompliance
(infraction: failure to analyze shoreline silt sample #5 for the second
half of 1978 (Paragraph 2)) was identified in one area.

Licensee Action on Previous Inspection Findings
a.
(Closed) Deviation (50-266/78-23 and 50-301/78-24): Failure to
maintain up-to-date and accurate emergency call list in the Two
Rivers Community Hospital's copy of the licensee's emergency plan.
The licensee's corrective action was to provide for the updated

list in this emergency plan copy. On November 28, 1979, the in-
spector examined the hospital's copy and confirmed that the up-
dated call list was present. The inspector has no further questions
regarding this matter.
b.
(Closed) Unresolved Item (50-266/79-15 and 50-301/79-17): Failure
to conduct an analysis for gross beta activity and gamma scanning
of the shoreline silt sample #5 for the second half of 1978. The
licensee reviewed his records and found no analytical results were
available of this particular sample. The sample, however, was col-
lected in October 1978 and shipped to the licensee's analytical
contractor for analysis. Failure to conduct the above analyses
represents noncompliance with Technical Specification 15.4.10.1.
The licensee's corrective action is to maintain and improve manage-
ment control over the licensee's contractor to assure required
analyses are performed.
3.
General
This inspection consisted of an examination of the licensee's quality
control in the confirmatory measurements, nonradiological environmental
monitoring program, and environmental effluent limits, and emergency
planning activities. The confirmatory measurements inspection included
completion of the review of the quality assurance and quality
analyticalmeasurementsdiscussedinthepreviousinspection.37ontrolof
A spiked
particulate filter and charcoal adsorber were also provided the licensee
for analysis.
The environmental protection inspection included a revieu of the licensee's
nonradiological effluent and environmental monitoring prcgram, including
sampling equipment and program results. Management controls, including
organizational structure, assignment of responsibility and authority,
and quality assurance audits, were reviewed. The emergency planning
portion of the inspection involved examination of cocrdination with
offsite support agencies, facilities, equipment, and supplies, medical
arrangements, training, emergency plan and procedures, and emergency
tests and drill exercises.
4.
Licensee Internal Audits
The inspector examined licensee review and audit activities in the area
of emergency planning, chemistry and radiochemistry, and environmental
monitoring. Audit reports and meeting minutes of the Offsite Review
Committee (OSRC) for 1978 to date of this inspection were reviewed.
Items which the licensee identified for corrective action included the
continuing problem of the intake crib freezing and updating of the
emergency plan call list. The inspector determined that corrective
action had been accomplished or instituted for these items. The in-
spector inquired as to the review and audit frequency and content of
environmental monitoring and chemistry and radiochemistry programs to
be conducted by OSRC in accordance with technical specification conditions.

The licensee agreed to discuss these matters at the next meeting of
the OSRC. This item will be examined during a subsequent inspection.
5.
Continuation of Confirmatory Measurement Inspection
a.
Licensee Program for Quality Assurance and Quality Control of
Analytical Measurements
The inspector completed the review of licensee procedures, logs,
check sheets, and records relating to nonradiological and radio-
logical analytica measurements which was started during a pre-
viousinspection.-}j
The inspector determined that the analytical
equipment in the laboratory and the gamma spectrometer and other
counting equipment had been adequately calibrated on the prescribed
frequency established in the licensee's procedures. The licensee
utilizes radioactive sources traceable to the National Bureau of
Standards for calibration of the counting equipment and spectrometer.
No technical weaknesses were identified during the examination of
the records and tour of the analytical laboratory.
b.
Spiked Samples
The inspector provided the licensee with a spiked air particulate
filter and charcoal adsorber for analysis. The licensee results
of the analytical measurements inspection will be compared with
the known values reported in a future confirmatory measurement.
No apparent items of noncompliance or deviations were identified.
6.
Management Controls
The licensee administrative and procedural controls for implementation
of the nonradiological effluent and environmental monitoring program
were reviewed.
Plant personnel conduct the nonradiological effluent
measurement program. The responsicility of conduct of the nonradio-
logical ecological monitoring program, including data evaluation and
report preparation, lies with the Corporate Nuclear Project Office.
The licensee's PBNP Environmental Manual serves to define and delineate
the responsibilities of the conduct of the environmental program. The
inspector reviewed this document, which includes procedures for sampling
and measurement of nonradiological and radiological samples. Audit
responsibilities for technical review and evaluation of data are also
assigned in this manual. No audit reports were available at the plant
during this inspection. This item will be examined during a subsequent
inspection.
7.
Quality Control of Analytical Measurements
The licensee's PBNP Environmental Manual describes the QA/QC program
used in the licensee's environmental program. The inspector reviewed
records for CY-1978 and 1979 to the date of this inspection of quality

control checks of the nonradiological ef fluent environmental program.
These included temperature instrumentation and recorder calibration
controls. No problems were identified.
During a tour of the plant, the inspector observed a number of liquid
effluent monitors such as pH and conductivity meters which were found
to be functional and calibrated currently. Selected checkoff sheets
for daily and monthly liquid effluent measurements for CY-1978 and 1979
to the date of this inspection were also reviewed to assure management
review of results occurred.
Calibration records for air sampling equipment used in the radiological
environmental monitoring program for calendar year 1978 and 1979 to date
of this inspection were reviewed.
The air samplers are periodically cali-
brated and checked for proper operation. No technical weaknesses were
identified.
No apparent items of noncompliance or deviations were identified.
8.
Implementation of the Nonradiological Environmental Monitoring Program
The licensee's five-year nonradiological environmental surveillance pro-
gram was completed during October 1977. On November 4, 1977, the licen-
see obtained an amendment to Technical Specification 16.2 to allow temp-
orary suspension of the nonradiological environmental monitoring program,
pending an assessment of the program.
In July 1978, the licensee sub-
mitted a summary report for this program to the Office of Nuclear
Reactor Regulation (NRR) for evaluation and review.
The program in-
cluded physical studies, water chemistry, bacteriology, phytoplankton,
periphyton, zooplankton, benthic macro-invertebrates, and fisheries.
The study concluded that very few effects of plant operation on the
physical, chemical and biological enviroments were revealed.
The NRR is preparing an enviroment impact assessment of the study which
is to be issued in the first quarter of 1980. This item will be exa-
mined during a future inspection.
No apparent items of noncompliance or deviations were identified.
9.
Nonradiological Effluent Releases
The inspector examined the licensee's records and semiannual reports
for thermal and chemical discharges and fish impingement for calendar
year 1978 and 1979 to date of this inspection.
a.
Chemical Effluents
The chemical effluent records and reports indicated that all
chemicals from the waste neutralizing tank, sewerage treatment
plant, and retention pond were within the Technical Specifica-
tion limits. The inspector found no instances of unmonitored
releases. No chlorine discharges have been made from the

plant since June, 1977 when the plant chlorinator unit was re-
moved from service.
b.
Thermal Effluents
During the review of licensee thermal effluent records, the in-
spector found no instances where Technical Specification limits
for condenser cooling water discharge temperature, differential
temperature or rate of change in temperature were exceeded during
normal plant operation. The inspector also determined that during
periods of winter in 1978 and 1979, there were several occasions
where the circulating water outlet temperature exceeded 89
F., the
Technical Specification temperature limit for normal plant operation.
However, in those cases examined, the plant was being operated in
the ice melt mode. This mode of operation to prevent ice buildup
of the intake crib does not constitute normal plant operation as
defined in Technical Specification 16.2.
Examination of the tem-
perature data in the semiannual reports for 1978 also indicated
no apparent problems.
c.
Fish Impingement Monitoring
The inspector examined the licensee's fish impingement moni-
toring records for CY 1978 and 1979 to date of this inspection.
No instances were found to indicate a significant fish kill in
excess of twenty fish, a limit used by the licensee (with Region
III concurrence), had occurred.
The inspector also observed the
callection of fish in the fish basket.
The licensee representative
identified the fish, and measured the length and weight of the fish.
No problems were identified.
No apparent items of noncompliance or deviations were identified.
10.
Licensee's Emergency Plan and Emergency Operating Procedures
The inspector discussed the emergency plan updating and review of
emergency procedures with licensee personnel. The current revision
(Revision 10, dated April 1979) was used as the basis for this in-
spection.
During a tour of the plant and visits to offsite support agencies,
the inspector observed that each copy of the emergency plan contained
this revision 10., including an updated emergency call list. At the
time of this inspection, the licensee was in the process of preparing
another updated call list.
The inspector reviewed the licensee emergency operating procedures
and noted that the licensee had prepared procedure E0P-9D, Revision 2,
datedNovember8,1979, entitled"EmergencyWeatherCongytions"in
response to an item discussed in a previous inspection.-
Review of
this procedure indicated it to be adequate.

11.
Coordination with Offsite Support Agencies
The inspector reviewed records, procedures, and written agreements
for CY-1978 and 1979 to date relating to the licensee's coordination
of emergency planning with eleven agencies and individuals listed in
the Emergency Plan and procedures. The inspector discussed this sub-
ject with licensee representatives and individuals either in person,
or by telephone, identified in Paragraph I from offsite support agencies.
The inspector also reviewed ten letters of agreement the licensee had
renewed with support agencies during 1979.
These letters are presently
being reviewed on an annual basis. These discussions verified that the
existing agreements bet.v.. the licensee and the offsite agencies remain
in effect and that the licensee's contact and coordinations were adequate
for these agencies to maintain an effective response capability.
No apparent items of noncompliance or deviations were identified.
12.
Facilities, Equipment, and Supplies
During this inspection a selective inventory of the licensee's first
aid room, and emergency supplies in the control room, the plant site
boundary control center, and other sele-+ed areas of the plant were
examined. The inspector also inspectea the plant emergency vehicle.
This examination verified that the items specified in the licensee's
emergency plan and procedures were available for use and maintained
in an operable state.
The inspector examined instruments to measure and record the magnitude
of radioactive releases during a tour of the control room. This in-
cluded meteorological instruments to measure wind speed and dir+ction.
Other decisional aids, such as maps for evacuation and isople'.hs for
dose assessment, were also examined. All equipment was found operable,
functioning properly and calibrated recently.
The inspector reviewed licensee inventory records of emergency supplies
and equipment for calendar year 1978 and 1979 to date of this inspection.
In addition, calibration records of emergency equipment and tests of
respiratory equipment were also reviewed.
No apparent items of noncompliance or deviations were identified.
13.
Medical Arrangements
The inspector reviewed records, procedures, and written agreements with
the Two Rivers Community Hospital and the Doctors Clinic for CY-1978
and 1979 to date. The inspector examined the special emergency hospital
room reserved for licensee personnel. The inspector took an inventory
of equipment and supplies maintained by the licensee. No items were
missing.
The inspector also examined the emergency vehicles maintained by the
Two Rivers Fire Department. These can be used as backup to the licen-
see's emergency vehicle.

14.
Training
The inspector reviewed training related documentation and procedures
for CY-1978 and 1979 to date to verify that training required ty the
emergency plan had been conducted. The licensee's training program
for onsite personnel consists of orientation sessions for new personnel,
including emergency planning activities, and first aid training for
selected individuals. The licensee utilizes the American Red Cross multi
media first aid program for their training program. The licensee also
conducts discussions of emergency plan related matters at routine plant
safety meetings.
The licensee also conducted annual training sessions for offsite per-
sonnel.
During a discussion with the nurses at the hospital and the
Doctors Clinic, the inspector found that the nurses would like to
participate in an emergency drill involving a simulated cont.minated
injured person.
Dr. Weld of the Doctors Clinic, also indicated that
additional training of the hospital staff would be desireable. This
item was discussed at the exit interview.
No apparent items of noncompliance or deviations were identified.
15.
Emergency Drills
The inspector reviewed records of emergency drills and test exercises
conducted since the previous emergency planning inspection. Drills were
held twice a year. Evacuation drills were conducted in December 1978
and May 1979
The drill in May 1979 was described in a previous inspec-
tion report.4j These drills also included communication checks of offsite
support agencies.
The inspector reviewed drill critiques prepared by the licensee and
determined the licensee had corrected any problems identified in the
drills.
No apparent items of noncompliance or deviations were identified.
16.
Exit Interview
The inspector met with licensee representatives (denoted in Paragraph
1) at the conclusion of the inspection on November 30, 1979. The in-
spector discussed the scope and purpose of the inspection and the
findings. With regard to the item of noncompliance related to the
radiological environmental sample, the licensee said they would improve
the control of analytical measurements of their contractor. The licen-
see also stated that when the Offsite Review Committee meets in December,
the subject of audit frequency and content of the environmental protection,
chemistry, and emergency planning activities would be discussed.
