V. C. Summer Nuclear Station Unit #1 Response to NRC Audit Report 50-395/79-01 dated February 5, 1979 Centlemen:
In-response to the above captioned report, we have reviewed the informa-tion contained there-in and find that it contains no proprietary information.
In addition, we have evaluated the circumstances relating to the item identified as 79-01-01 in report 50-395/79-01, which dealt with failure to follow procedures for pipe support installation. Our evaluation has produced the following results:
1.
Cause a)
Failure to comply with procedure AP-III-02 as written at that time.
The hanger engineer did instruct the field as to how to handle the problem, and so noted it in the packet by memo. An FCR-B was to follow but the hanger engineer failed to initiate it.
This " unofficial" method of control developed over time as a practical expedient to supplement and preceed issuance of the FCR-B.
b)
Craft submitted incompleted snubbers to QC for inspection.
c)
Failure of QC to inspect to approved Quality Control Procedure.

Immediate Corrective Steps Taken and Results NCN 0588 was issued to document the prceedural violation and to request an FCR be written to document changes made to CCH-386 by the engineer. FCR-B 5106-ASME has been initiated.
The responsible QC Inspector has corrected entries in packets CCH-386 to reflect the lack of an FCR and in packets RHH-099 and RifH-100 for not having set screw lock wires. Packets RHH-099 and 100 have been returned to the field to have the work completed. Investigation of the matter indicates this is an isolated case.

Corrective Steps Taken to Avoid Future Noncompliance Responsible piping hanger engineer has been cautioned to comply with the requirements of Procedure AP-III-02.
The re-spective craft personnel have been cautioned by the Piping Manager not to proceed with work without the proper authorization and to assure snubbers are complete before submitting to QC.
The responsible QC Inspector has been cautioned to ascertain the correctness of documentation and drawings to preclude inaccurate entries and improper verification, and to assure the correct installation of the hardware.
Final inspection by QC will verify that the FCR-B is included in the hanger packet.
Procedure AP-III-02 was revised by an IAP on 12/4/78, to incorporate the good aspects of the " unofficial" method and allow more expeditious processing of hanger modifications without de-grading control. The revision of the procedure will help to prevent recurrance.

Full Compliance Date All actions identified were complete as of this date.
