S APPENDIX A LE Gi VIOLATION South Carolina Electric and Gas Company License No. CPPR-94 Based on the results of the NRC inspection conducted on January 9-12, 1979, it appears that certain of your activities were not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in our correspondence to you dated December 31, 1974.

As required by Criterion V of Appendix B to 10 CFR 50, as imp'.emented by South Carolina Electric and Gas Company (SCE&G) FSAR Section 17.1.5,

" Activities affecting quality shall be prescribed by documented instructions, procedures or drawings...and shall be accomplished in accordance with these instructions, procedures or drawings".

Paragraph 4.5 of Daniels Construction Company (DCC) QC Procedure VII-07 states, " Verify that the requirements of this procedure and the requirements of the installation detail drawing and checklists have been met, and indicate acceptance by signing in the appropriate space on the hanger and snubber installation checklists".

Paragraph 3.1 of DCC Administrative Procedure III-02 states, "The Field Change Request (FCR) form, exhibit A, sball be used to document requests to deviate from design documents, and the response thereto".

Contrary to the above, although hanger CCH-386 had not been installed in the configuration shown on the detail drawing, no FCR had been issued for the alterations and the hanger installation checklist had been signed by the QC inspector verifying that the installation was as shown on the hanger drawing.

In addition, the snubber installation checklists for snubbers RHH-99 and RHH-100 were signed by the QC inspector indicating that the extension piece to snubber body mounting bolts had been locked (lockwired or threads upset) and that lockwiring of dust cover set screws was not applicable.

The fasteners on these snubbers were not locked when observed on January 11, 1979.
