APPENDIX A NOTICE OF VIOLATION Tennessee Valley Authority License No. CPPR-73 Based on the NRC inspection January 30 through February 1,1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below.

These items have been categorized as described in correspondence to you dated December 31, 1974.

A.

As required by 10 CFR, Part 50, Appendix B, Criterion V, as implemented by the FSAR, paragraph 17.1. A.5, " Activities affecting quality shall be prescribed by documented instructions, procedures.... and shall be accomplished in accordance with these instructions, procedures....".

TVA Process Specification Number 4.M.1.1(b) dated December 20, 1973 contains the following requirements:

1.

Paragraph 3.1.10.1, "End caps for tubular products shall be main-tained in place".

2.

Paragraph 3.1.10.2, "When stainless steel material or components are stored indoors, sufficient protection from shop dust, grinding particles, machining chips, etc., shall be provided to maintain cleanliness".

3.

Paragraph 3.1.12.1, " Carbon steel fixtures, slings or handling rigs shall not contact stainless steel components for safety-related systems".

Contrary to the above, on January 31, 1979 stainless steel containment spray piping located in the containment dome area had the following:

1.

Pipe end caps missing-three examples.

2.

Uncovered wire rope around pipes-two examples.

3.

Pipe placed against unpainted / uncovered carbon steel stands-four examples.

4.

Spray nozzle bosses uncovered-approximately one-third of bosses obse rved.
