Appendix A NOTICE OF VIOLATION The William Powell Company License No. 34-02963-01 As a result of the inspection conducted on November 20, 1980, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:
1.
Condition 8D of your license limits the amount of cobalt-60 that you may possess in Technical Operations Model 446 exposure devices to a total of 3 curies per source.
Contrary to this requirement, on the day of the inspection, you possessed 8 curies of cobalt-60 (source model A424-8, serial No. 1770) in a Technical Operations Model 446 exposure device.
This is a Severity Level IV violation (Supplement VII).

10 CFR 34.43(c) requires that a record be maintained of the final survey made to determine that the sealed source is in its shielded position prior to locking the exposure device.
Contrary to this requirement, one of your radiographers failed to record the results of final surveys performed on August 7, 8, October 20 and 21 of 1980.
This is a Severity Level VI violation (Supplement VII).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within twenty-five days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under o-th or affirmation.
