Dear Mr. McClintock:

In response to your letter of January 15, 1979 with respect to items of non-compliance found by the U.S. Nuclear Regulatory Commission inspection of December 28, 1978, please be advised of the following:

1) Chart Location - Dr. Krauthamer will have all the charts on patients l

in whom pacemakers were implanted returned to the I

Cardiology Department by February 2, 1979.

These charts'will remain within the Department.

2) Chart Completeness - Dr. Krauthamer will have the charts completely up-dated as per my previous recommendations as well as the requirements for data as detailed in the original protocol submitted to the U.S.N.R.C. by February 2, 1979.

Audits - Mr. James Celsomino, Physicist Assistant or Mr. Carl Collica, Physicist will conduct audits of the patients charts on a quarterly basis.

4) Material Transfer - Mr. Gelsomino or Mr. Collica will be called to oversee that receipt and transfer of the Special Nuclear Materials is in accordance with the require-ments of the several regulatory agencies involved.

Therefore, the corrective steps taken to achieve compliance would be Items 1 and 2.

The corrective steps taken to maintain compliance would be Item 3 and 4.

The date that full compliance will be achieved should not be later than February 28, 1979.

f In addition, an audit of the Nuclear Pacemaker Program will be conducted on an annual basis with a representative of Management prior to March 9, 1979.

The results of all audits will be retained in Radiology with copies in l

Management and Cardiology.
