l Norwalk, Connecticut 06856 License No. Stim-1504 Based on the results of an NRC inspection conducted on December 28, 1978, it appears that one of your activities was not conducted in full compliance with NRC regulations and the conditions of your license as indicated below:
a Condition 15 of your license requires that licensed material be possessed and used in accordance with statements, representations and procedures contained in certain referenced applications and letters.
One of these letters is dated November 7,1974.
Paragraph 3 of this letter requires that you establish and maintain l
in your hospital, a file of pertinent information for each patient who underwent a pacemaker implantation in your hospital.
Contrary to this requirement, as of the day of the inspection, December 28, 1978, the files established in your hospital did not l
contain the required pertinent information for each patient.
This item is a deficiency.
