TOLEDO EDISON COMPANY DAVIS-BESSE NUCLEAR POWER STATION UNIT ONE SUPPLEMENTAL INFORMATION FOR LER NP-33-79-21 DATE OF EVENT: January 29, 1979 FACILITY: Davis-Besse Nuclear Power Station Unit 1 IDENTIFICATION OF OCCURRENCE: Containment Spray Pump 1-2 suction and discharge valves not opened during startup Conditions Prior to Occurrence:

The unit was in Mode 1, with Power (MWT) = 153, and Load (Gross MWE) = 0.

Description of Occurrence: On January 29, 1979 at 1015 hours0.0117 days <br />0.282 hours <br />0.00168 weeks <br />3.862075e-4 months <br />, an equipment operator noted the suction and discharge valves on Containment Spray Pump 1-2 were closed.

This placed the unit in the Action Statement of Technical Specification 3.6.2.1, which requires operability of two independent Containment Spray Systems during Modes 1 through 4 operation. At 1030 hours0.0119 days <br />0.286 hours <br />0.0017 weeks <br />3.91915e-4 months <br /> on January 29, 1979, the suction and discharge valves were opened and Containment Spray Pump 1-2 was vented. This removed the unit from the Action Statement of Technical Specification 3.6.2.1.

Designation of Apparent Cause of Occurrence: The cause of this occurrence is attri-buted to personnel error. The suction and discharge valves had been tagged shut for maintenance purposes during the preceding outage. The operator who removed the tags failed to ensure the Containment Spray Pump was properly lined up.

Additionally, an error regarding which type of maintenance was actually performed resulted in deletion of testing requirements which would have detected the Lsproper lineup.

Analysis of Occurrence: There was no danger to the health and safety of the public or to unit personnel. Containment Spray Pump 1-1 was operable.

The, integrity of the Reactor Coolant System remained intact and the relatively low reactor oower would have minimized e.he effect of a loss of coolant accident during the time span when the Containment Spray Pump 1-2 was inoperable.

Corrective Action

Upon discovery of the inoperable status of Containment Spray Pump 1-2, the pump suction and discharge valves were opened.

The pump was vented and restored to an operable status.

A procedural revision (M-2630) to the Plant Startup Procedure, SP 1102.02, has been prepared which requires the performance of valve lineups and specified testing dur-ing plant startup. The event was reviewed with the involved personnel.

Failure Data:

On January 12, 1978, and December 28, 1978, the unit entered Mode 4 without restoring the Containment Spray Pumps to operable status (see Licensee Event Reports NP-33-78-10 and NP-32-78-13).
