O'Reilly - February 19, 1979 IE INSPECTION REPORT 50-369/78-44 50-370/78-18 APPENDIX A NOTICE OF VIOLATION Duke Power Company License No. CPPR-84 Based on the results of the NRC inspection conducted on December 27-29, 1978, it appears that certain of your activities were not conducted in full com-pliance with NRC requirements as indicated below. These items have been categorized as described in our correspondence to you dated December 31, 1974.

Criterion V of Appendix B to 10 CFR 50, as implemented by DPC Topical Report " Duke 1-A" Section 17, Paragraph 17.1.5 specified in part the

" Activities affecting quality shall be accomplished in accordance with the requirements imposed by.

. instructions, procedures and drawings."

QA Procedure H-3 Rev. 9, paragraph 5.7.1 states in part that "... If all the welding material is not used in the time allotted in Table 1, the remaining material and corresponding H-3B forms shall be returned to the issue station." Table 1 requires E7018 weld rod to be returned to the issue station after 4 1/2 hours maximum exposure.

Contrary to the above, on December 28, 1978, E7018 weld rods were located on "I" beams and in trash cans in the Reactor Building.

This is an infraction.

Response

Welding supervision and craft personnel have been reinstructed as to the proper manner of disposing of unused welding material as required by QA Procedure H-3.

The E7018 weld rods located on "I" beams and in trash cans were returned to the issue station and a subsequent survey of the Reactor Building has revealed no further problems.
