L-2019- IN - Strontium-82/Rubidium-82 Generator Breakthrough Events and Issues 1 08/2019 GEN-0010 The purpose of this IN is to inform addressees of incidents where excessive levels of Strontium-82 and Strontium-85 were unknowingly administered to patients during a cardiac imaging procedure because of failed Strontium-82/Rubidium-82 generators.
L-2019- RIS - Clarification of Two-Person Rule for Industrial Radiography 1 09/16/2019 GEN-0011 The U.S. Nuclear Regulatory Commission (NRC) is issuing this regulatory information summary (RIS) to provide an alternative regulatory interpretation of NRC surveillance requirements to allow the second qualified radiographer to perform other tasks in another area while still being able to meet the surveillance requirement to provide immediate assistance.
L-2019- IN - Recent Reported Medical Events Involving the Administration of Yttrium-90 Microspheres for Therapeutic 1 09/16/2019 GEN-0012 Medical Procedures The U.S. Nuclear Regulatory Commission (NRC) is issuing this information notice (IN) to inform licensees of recent reported medical events that involved the administration of Y-90 microspheres. Medical events described in this IN are attributed to written directive errors, clogging or kinking of the delivery system, and treatment of the wrong location.

SUMMARY
Date Opened Date Closed L2018- IN Emergency Diesel Generator Excitation System Diode Failures 09/10/2018 06/05/2019 GEN-0007 EDG excitation system diode failures continue to occur. This information notice is intended to inform addressees that these conditions can lead to demand/run failures and can adversely affect the ability of EDGs to perform their safety functions.
