JUN 151975 APPENDIX A NOTICE OF VIOLATION Georgia Power Company License Nos. DPR-57 Hatch I and 2

& NPF-5 Based on the NRC inspection May 7-11, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.

A.

As required by 10 CFR 20.201(b) each licensee shall make or cause to be made such surveys as may be necessary for him to comply with the regulations in this part.

Contrary to the above, the licensee did not perform surveys necessary to identify and properly post a high radiation area near a section of residual heat removal (RHR) system piping located in the RHR room on the 87 foot elevation of the Unit 1 Reactor Building. On May 7, 1979, radiation levels in the area were 175 mR/Hr.

This is an infraction.

B.

As required by Technical Specification 6.13.1 (Unit 1) and 6.12.2 (Unit 2) each high radiation area in which the intensity of radiation is greater than 1000 mR/Hr shall be provided with locked door to prevent unauthorized entry into such areas.

Contrary to the above, on May 7,1979, the. door leading to the Unit I reactor water cleanup heat exchanger room (door R32) was found to have part of the wire mesh in the area of the lock missf,g. This permitted the door to be opened from t6e outside without n key, thus s11owing unauthorized entry into the area. Radiation levels in accessible areas of the room were 1250 mR/Hr.

This is an infraction.

C.

As required by Environmental Technical Specificati n a report on the environ-mental surveillance program for the previous calendar year shall be submitted to the NRC within 90 days after January 1 of each year.

Contrary to the above, the radiological section of the annual Environmental Surveillance Report for 1978 was not submitted to the NRC within 90 days af ter January 1,1979. The report had not been submitted on May 11, 1979.

This is a deficiency.
