Report Date October 30, 1978 Occurrence Date October 17, 1978 Identification of Occurrence Discovery of a crack in the core spray sparger piping. This event is con-sidered to be a reportable occurrence as defined in the Technical Specifi-cat ions, paragraph 6.9.2.a. (9).

Conditions Prior to Occurrence The plant was in a refueling shutdown. Reactor coolant less than 212*F.

Description of Occurrence On Monday, October 16, 1978, at approximately 1800 hours0.0208 days <br />0.5 hours <br />0.00298 weeks <br />6.849e-4 months <br />, an indication was discovered on the core spray sparger piping for System II.* Location of the indication was at an azimuth location of 208* in the reactor vessel.

From initial investigation, the indication appears to extend at least 180' circumferential1y around the pipe. Further investigation on October 19, 1978, established that there is a through wall crack for about 135' of the pipe's circumference.

Apparent Cause of Occurrence Investigations into the cause of the crack have determined that the most probable cause was the methods used to install the spargers.

Reportable Occurrence No. 50-219/78-21/1T-0 Page 2 October 30, 1978 a complete break occur. A complete break and separation of the piping could reduce the capability of the core spray system Il to adequately wet the core during a LOCA condition. Further investigation of the indication and analysis of the consequences will provide for a more firm analysis of the occurrence.

Corrective Action Corrective action will be determined after a full investigation is complete.
