,' ENCLOSURE 1 Response To NRC-0IE Letter From C. E. Murphy To H. G. Parris Dated February 8, 1979 (Reference RII:JJL 50-518/79-01, 50-519/79-01, 50-520/79-01, 50-521/79-01)

This report responds to the following Notice of Violation described in Appendix A of IE Inspection Report RII:JJL 50-518/79-01, 50-519/79-01, 50-520/79-01, and 50-521/79-01.

Noncompliance Item - Infraction - 521/79-01-01 As required by Criterion V of Appendix B to 10CFR50, as implemented by PSAR Section 17.1A.5, " Activities affecting quality shall be prescribed by docamented instructions, procedures, . . . and shall be accomplished in accordance with those instructions, procedures . . ." TVA Specification G-2 and C. F. Braun Specification 300-1 requires that concrete surfaces be protected from loss of moisture for e minimum of fourteen days following placement.

Contrary to the above, on January 3,1979, at 9:00 a.m. and at 4:30 p.m.,

and on January 4, 1979, at 7:30 a.m., large areas on the surface of pour number BZR-1A were not protected from loss of moisture. Pour number BZR-1A was placed on December 21-22, 1978.

This is an infraction.

Response

The above condition was identified on January 4, 1979, on Quality Control Information Report (QCIR) No. 3822. To correct the condition, pour B2R-1A was cured three additional days as noted in the QCIR disposition dated January 17, 1979. .

To prevent a recurrence of this condition, a memorandum was written February 21, 1979, from Plant B Construction Engineer, W. T. Quinn, and Plant A Construction Engineer, W. R. Brown, to Construction Superintendent, L. A. Wilson, reemphasizing the proper cold weather protection procedures as specified by Quality Control Instruction (QCI)-202 and General Construction Specification G-2.

This report responds to the following Notice of Deviation described in Appendix B of IE Inspection Report RII:JJL 50-518/79-01, 50-519/79-01, 50-520/79-01, and 50-521/79-01.

Noncompliance Item - Deviation - 518/79-01-02, 519/19-01-02, 520/79-01-02, 521/79-01-02 The response to PSAR Question Number 130.23 states that, unless otherwise specified, placement, inspection and testing of concrete would be in accordance with requirements of ANSI N45.2.5. ANSI N45.2.5-1974 was the current edition of the Standard at the time the response to the question was made and SER Section 3.8.4 designates ANSI N45.2.5-1974 as the effective revision of the standard upon which evaluation of the construction program was made.

ANSI N45.2.5-1974 requires that pumped concrete must be sampled at the pump line discharge. No exceptions were taken to the requirement in response to PSAR Question 130.23.

Contrary to the above, concrete was not sampled at the pump line discharge as required by ANSI N45.2.5-1974. This is a deviation.

Response

The sampling of pumped concrete requirements of ANSI N45.2.5-1974, is mot included in the present revision of Quality Control Instruction (QCI)

C-208, " Concrete Compressive Strength, Slump, Air Content, and Unit Weight."

Until January 12, 1979, sampling was done in accordance with QCI C-208.

Upon identification of this nonconformance, Quality Control Information Report (QCIR) Hos. KNP-A-5880 and HNP-B-3851 were generated. These resulted in the preparation of NCR HNP-A-035 on January 22, 1979. The NCR requested that the concrete in place should be accepted based on the samples taken for other purposes which indicated acceptable correlation. The request was accepted by P. V. Scanlan of C. F. Braun and J. E. Nichols of General Electric Company on February 8, 1979.

To prevent recurrence of this condition QCI C-208 will be revised to require sampling to be, performed at the pump line discharge per ANSI N45.2.5-1974 for as.y Category I pour being placed by pumping. Until this change is incorporated, site personnel have been instructed to follow the requirements of ANSI N45.2.5-1974.
