A.
Technical Specification 6.8.1 requires the licensee to establish, maintain, and implement procedures governing maintenance on safety-related systems.
HtP-8, Maintenance Reouest provides requirements for effecting timely maintenance on systems where plant safety is impaired and/or plant shutdown will be required.
Contrary to the above, a time delay relay f undamental to design operation of the low pressure coolant injection system was f ound inoperable on April 12, 1978, and not repaired until June 17, 1978.
During this time the plant was operated at power on several occasions.
This item is an inf raction.
B.
Technical Specification 6.9.1.2.b provides requirements for submitting a written report within 30 days when conditions leading to operation in a degreded mode permitted by a limiting condition for operation are identified.
Contrary to the above, the inoperable time delay relay identified on April 12,1978, and repaired on June 27, 1978, which resulted in a degradation of the operability of an RHR pump and its essociated diesel generator, was not reported to the NRC.
This is a deficiency.
