We offer the following responses to the apparent Items of Noncompliance in the referenced Inspection Report.
NOTICE OF VIOLATION As required, in part, by Appendix B, Technical Specifications 3.1.4.a and 3 1 4.b, oxygen saturation will be measured at dusk and dawn of consecutive days, twice monthly in the outer bay, intake canal, and discharge canal at the point of discharge.
Contrary to the above, the licensee failed to measure oxygen saturation at the assigned f reque.cy during January and February, April and June 1978, in the case of the cater bay, and January and June 1978, in the case of the ca na l s.
Response
During 1977 and 1978, oxygen saturation was not tueasured as requi' ed by Appendix B, Technical Specifications 3.1.4.a and 3.1.4.b, f or the dates, at the stations, and for the reasons stated in Table 1.
Of the 564 uasurements that were scheduled tc be obtained, 11 measurements were missed because of equipment failures, 3 measurements were missed because of adverse weather conditions, and 37 measurements were missed because of personnel misunderstandings.j} l}} The contractor did not understand the seriousness of these measurements as a Technical Specification requirement until the FPC audit in June, 1978. Prior to this audit, the contractor felt that other similar measurements (major diurnals) would be adequate in place of the missed readings. Measurements missed because of equipment failures are undesirable, but not totally unavoidable.
