80-006/03L-0 EVENT DATE: 1-15-80 TITLE OF REPORT: Liquid Waste Release 1.

DESCRIPTION OF EVENT

During routine surveillance of 11guld waste release from 1-LW-TK-5B, it was discovered that the circulacion pump 1-LW-P-5A between the two tanks was inadvertently left on.

This is contrary to T.S. 3.ll.A.4 and is re-portable per T.S. 6.6.2.b.2.

PROBABLE CONSEQUENCES AND STATUS OF REDUNDANT SYSTEMS:

The activity of liquid waste in both tanks was low because it was properly processed, and was ready for release. During the event, liquid waste mon-itor RM-LV-108 was in service as required and the release rate was within the limits.

If the activity of the unsampled tank was high, it would have been detected by the radiation monitor and the release would have been ter-minated automatically. The total activity released was conservatively estimated to be 0.0023 C1 (2.63 x 10-2 % T.S.).

Therefore, health and safety of the public were not affected.

CAUSE

The circulation pump 1-LW-P-5A between the two tanks was inadvertently left on prior to initiating the release.

4.

IMMEDIATE CORRECTIVE ACTION

The release was terminated and 1-LW-TK-5A was sampled. The total volume of transfered liquid waste from tank 5A to 5B plus the difference betseen the total volume released and volume transfered were used to conservatively estimate the total activety relcssed.

SCHEDUI.ED CORRECTIVE ACTION:

Ncne required.

ACTION TAKEN TO PREVENT RECURRENCE:

Operators have been instructed to use approved procedures specifically designed to avoid recurrence.
