Heat Tracing System Failure 1.

DESCRIPTION OF EVENT

While conducting PT 27D (Heat Tracing System) it was discovered that circuit 17A Panel 9 was inoperative. The redundant heat tracing circuit on this line to !!o. 1 Boric Acid Filte: was operable thus maintaining two flow paths to the core. This event is contrary to Tech. Spec. 3.2.B.5, allowable as a degra-ded mode of operation as outlined in 3.2.D.3, and reportable in accordance with 6.6.2.b.2.

The health and safety of the public were not af fected.

2.

PROBABLE CONSEOUENCES AND STATUS OF REDU'IDANT SYSTEliS:

The redundant circuit from Panel 8 was operable and functioned as designed.

Since two flow paths for Boric Acid feed to the core were maintained operable, the health and safety of the public were not affected.

3.

CAUSE

Investigation determined that a thermostat within the circuit had failed.

4.

IIIMEDIATE CORRECTIVE ACTION:

A maintenance report was initiated to replace the defective part.

5.

SUBSEQUE!iT CORRECTIVE'ACTIO!!:

The defective thermostat was replaced and the system declared operable.

6.

FUTURE CORRECTIVE ACTION:

None.

7.

CENERIC IIIPLICATIONS:

A task force has been established to study the total spectrum of the heat tracing system in an attempt to discern present problems and recommend solutions which will eliminate the failures experienced.
