APPENDIX NOTICE OF VIOLATION Duke Power Company License No. CPPR-83 McGuire Based on the NRC inspection August 16-23, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
As required by Criterion V of Appendix B to 10 CFR 50, as implemented by Topical Report, Duke 1A, paragraph 17.1.5, " activities reflecting quclity shall be prescribed by documented instructions, procedures...
and shall be accomplished in accordance with these instructions, procedures..."
Contrary to the above, on August 23, 1979, the inspector found vent valve DC331 on penetration M255 closed and signed off open when required to be open as per procedure TP/1/A/1200/06, The Containment Initial Integrated Leak Rate Test and Structural Integrity Test.
This is an infraction.
