MCGUIRE NUCLI".R STATION Response to IE Inspection Report 50-369/79-29 NOTICE OF VIOLATION Duke Power Company License No. CPPR-83 McGuire Based on the NRC inspection August 16-23, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. '

As required by Criterion V of Appendix B to 10CFR50, as implemented by Topical Report, Duke 1A, paragraph 17.1.5, " activities reflecting quality shall be presecribed by documented instructions, procedures...

and shall be accomplished in accordance with these instructions, procedures..."

Contrary to the above, on August 23, 1979, the inspector found vent valve KC331 on penetration M355 closed and signed off open when required to be open as per procedure TP/1/A/1200/06, The Containment Initial Integrated Leak Rate Test and Structural Integrity Test.

This is an infraction.

Response

Penetration M355 consists of four valves which must be properly aligned in order to conduct a leak rate test of the penetration (1KC331 open, and 1KC280, 1KC332B, 1KC333A closed). A retest of penetration M355 was con-ducted on September 6 and September 9, 1979 with a measured leakage of 0 secm. Valve 1KC331 was verified open, and the other three valves associated with this enetration were verified closed during this retest.

Subsequent to this IE inspection, valve INC103 associated witn penetration M216 was discovered to have been misaligned during the Containment Integrated Leak Rate Test. A retest of this penetration was conducted on September 6, 1979, with a measured leakage of 0 sccm.

These two identified valve misalignments were caused by personnel error.

Duke Power Company has reexamined the valve lineup and verified that the remaining valves were correctly aligned in accordance with procedure TP/1/A/1200/06, The Containment Initial Integrated Leak Rate Test and Structural Integrity Test.
