APPENDIX A NOTICE OF VIOLATION Jersey Central Power and Light Company Docket No. 50-219 Based on the re'sults of an NRC inspection conducted on October 17-20, 1978, it appears that one of your activities was not conducted in full compliance with conditions of your NRC Facility License No. DPR-16, as indicated below. This item is an infraction.
Technical Specification 6.8.1, states in part, " Written procedures shall be established, implemented, and maintained...."
P ocedure No. 205.6, Fuel Shuffle (Refueling) Revision 0, September 18, 1978, Paragraph 3.9, states, " cleanliness zone around reactor cavity, fuel pool, and separator / dryer cavity is established in accordance with reference 2.13."-
(Procedure No. 119, Housekeeping, Revision 2, September 1,1978)
Procedure No. 119 requires monthly, documented, inspections of the refueling area (more frequently during refueling activities) and of any system that is open or will be opened to prevent the introduction of foreign materials into the system.
Contrary to the above, paper, tape, and other debris were found floating in a stagnant area of the dryer / separator pool.
In addition, there was no documentation to provide support that the general area or pool area had been inspected.
